A pulsating, throbbing toothache indicates acute pulpal inflammation (irreversible pulpitis) or a localized periapical abscess. When bacterial contamination penetrates the closed pulp chamber, increased vascular pressure cannot expand through rigid dentin walls, compressing pulpal nerve endings and causing intense synchronized arterial throbbing.

What to Do Immediately
Rinse with warm salt water to help clear oral debris. Take doctor-approved over-the-counter analgesics (such as Paracetamol or Ibuprofen) with food. Keep your head elevated on pillows to reduce blood pressure in the head, and seek emergency dental treatment immediately.
⚠️ What NOT to Do
NEVER place an aspirin tablet or crushed painkiller against the gums (causes severe chemical burn ulcers). Do not apply external heat pads, which accelerates infection spread into facial tissue spaces.
