Wisdom Tooth Pain — Causes, Home Relief & When to See a Dentist in Indore
There are few things as disruptive as a severe toothache, and wisdom tooth pain is notoriously intense. It can radiate from the back of your jaw to your ear, throat, and head, making it impossible to eat, sleep, or concentrate.
If you are in pain right now, you need to know two things: how to get temporary relief, and when it is dangerous to ignore it. Dr. Shailly Nimbark from Dream Smile Dental Clinic, Indore, explains the causes of wisdom tooth pain and what you can do about it.
Why Does Your Wisdom Tooth Hurt?
Wisdom teeth (third molars) usually emerge between ages 17 and 25. Because they are the last to erupt, there is often not enough room left in the jaw. This lack of space causes the majority of the pain issues.
1. Pericoronitis (Gum Infection)
This is the most common cause of acute wisdom tooth pain. When a wisdom tooth only partially breaks through the gums, a flap of gum tissue (an operculum) rests over the tooth. Food debris and bacteria easily get trapped underneath this flap. It becomes severely inflamed, swollen, and infected. This condition is called pericoronitis. It causes intense, throbbing pain, bad breath, a bad taste, and difficulty opening the mouth.
2. Impaction
An impacted wisdom tooth is blocked from erupting fully and remains stuck in the jawbone. It may grow sideways, pushing against the roots of the adjacent second molar. This pressure causes a deep, aching pain in the jaw.
3. Severe Decay (Cavities)
Because they are far back in the mouth, wisdom teeth are incredibly difficult to reach with a toothbrush or floss. Plaque accumulates rapidly, leading to deep cavities. When the decay reaches the nerve (pulp) of the tooth, it causes acute, sharp pain.
4. Eruption Pain
Sometimes, normal eruption simply hurts. Just like a baby teething, a wisdom tooth pushing through the gums can cause dull, aching discomfort for a few days at a time as it slowly emerges.
Temporary Home Relief for Wisdom Tooth Pain
Important: These remedies provide temporary relief from symptoms; they do not cure an underlying infection or fix an impaction. You must eventually see a dentist.
- Warm Salt Water Rinses: The absolute best home remedy for pericoronitis. Dissolve half a teaspoon of salt in a glass of warm water. Swish it around the back of your mouth for 30-60 seconds, focusing on the painful area, and spit. The salt helps draw out fluid from swollen gums and creates a hostile environment for bacteria. Do this 4-5 times a day.
- Over-the-Counter Painkillers: Ibuprofen (Brufen) is an NSAID (non-steroidal anti-inflammatory drug) and is usually more effective for dental pain than Paracetamol because it reduces both pain and inflammation. Follow dosage instructions carefully.
- Cold Compress: If you have swelling in your cheek, apply an ice pack wrapped in a cloth to the outside of your face for 15 minutes at a time. This reduces inflammation and numbs the area.
- Clove Oil (Eugenol): Clove oil contains eugenol, a natural numbing agent and antibacterial. Apply a tiny drop of clove oil to a cotton swab and gently dab it on the painful gum area. Be careful not to use too much, as it can burn the surrounding tissue.
- Keep the Area Clean: Even though it hurts, you must try to keep the area clean to reduce bacterial load. Use an extra-soft toothbrush to gently brush the area, or use an antimicrobial mouthwash (like one containing Chlorhexidine).
Red Flags: When to See a Dentist Immediately
Do not rely on home remedies if you experience any of the following. These are signs of a spreading infection that requires professional medical intervention and antibiotics:
- Significant Facial Swelling: Swelling that extends to your cheek, neck, or under your jaw.
- Trismus (Lockjaw): Severe difficulty or inability to open your mouth.
- Fever and Chills: Systemic signs that an infection is spreading through your body.
- Difficulty Swallowing or Breathing: This is a medical emergency. Go to an ER or an oral surgeon immediately.
- Pus Drainage: A foul-tasting, yellowish discharge leaking into your mouth from the gum area.
- Pain that does not improve after 48 hours of painkillers and salt water rinses.
Professional Treatment at Dream Smile Dental Clinic
When you visit the clinic for wisdom tooth pain, we will first take an X-ray (OPG) to see the position of the roots and the extent of any infection. Treatment involves:
- Immediate Relief: If you have pericoronitis, the dentist will thoroughly clean and irrigate the area under the gum flap to flush out bacteria and trapped food. They may prescribe antibiotics and stronger painkillers to bring the acute infection under control.
- Definitive Solution (Extraction): Once the acute infection and swelling have subsided, the recommended treatment is usually the extraction of the problematic wisdom tooth. This prevents the infection from returning. See our Guide to Wisdom Tooth Extraction Costs and Procedure.
Frequently Asked Questions
Will antibiotics cure wisdom tooth pain?
Antibiotics will temporarily reduce an infection, easing the pain. However, they do not fix the physical problem (an impacted tooth, a gum flap trapping food, or a deep cavity). The infection will almost certainly return once the antibiotics are finished unless the tooth is treated or removed.
Is it normal for wisdom tooth pain to come and go?
Yes. Pericoronitis often flares up, causes severe pain for a few days, and then subsides as the infection temporarily drains or the immune system suppresses it. However, the flare-ups will continue to recur (often worsening each time) until the tooth is extracted.
Can I just cut away the gum flap myself?
Absolutely not. Attempting to cut or manipulate the gum tissue yourself can introduce massive amounts of bacteria, causing a severe, potentially life-threatening infection. Only a dentist should perform an operculectomy (surgical removal of the gum flap) if deemed appropriate.
